[Blink] Issues with incoming calls and BYE (mac)

There seems to be two interesting issues with regards to incoming calls  
and BYE.

First, on an incoming call when the hangup button is pressed, the Blink  
client doesn't send a BYE to the far end.  Nothing shows up in the log,  
nothing shows up on the wire.  Blink closes the call dialog, but never  
tells the far end.

Second, if the far end sends a BYE, Blink complains that the CSEQ is  
incorrect and rejects the BYE:

Leaving the call dialog up.

Strangely, both sending and receiving BYE works correctly if Blink  
originated the call, it only exhibits this behavior when it is the  
recipient of a call.
