[SIP SIMPLE client] Addition to SipTesting page

On http://sipsimpleclient.com/wiki/SipTesting, the Testing / Configuration 
heading described setting up a single account. By adding the following at the 
end of that section (just above the "Register" heading) it becomes clearer 
that there are more settings needed, in particular the outgoing proxy.

Each account has its own settings. Settings use a dotted hierarchy. You may 
need to make additional settings for each account in order to make outbound 
calls. A common configuration setting is the outbound SIP proxy (for instance 
if outbound calls never ring).

        sip-settings -a set user at domain sip.outbound_proxy=proxyname

If you have a sip2sip.info account, the outbound proxyname can be found on the 
<a 
href="http://wiki.sip2sip.info/wiki/SipDeviceConfiguration#DeviceConfiguration">sip2sip 
wiki</a> (usually proxy.sipthor.net).
